What we provide	The Aged Care Navigation Service provides Information on the aged care system Practical support to access aged care services in the home or in residential aged care Group information sessions about aged care
Eligible clients	ADA Australia’s Aged Care Navigator services are available to people residing in the Brisbane South and Bundaberg/Wide Bay regions, who need information about aged care options, or help to access aged care supports. This includes older people, their families, friends and others who want to link with aged care supports. The services are particularly aimed at helping people who: Live in rural or remote areas; Have a diverse cultural background; Have limited access to internet or other technologies; Identify as gender diverse; Are vulnerable, isolated, homeless or disadvantaged; Are experiencing dementia or poor mental health; Are unlikely to seek aged care support without assistance from others.
We can help with …	The Aged Care Navigation service supports, educates and assists people by informing and linking them with suitable aged care services. They aim to help local people build their own knowledge by guiding them through the steps from information to assessment to receiving care. Supports provided by ADA Australia’s Aged Care Navigation service may include Assistance with filling in forms Contacting My Aged Care Providing factsheets, handouts and other resources Outreach services Individual support sessions Group support sessions (Bundaberg/Wide Bay region only) Community workshops (Bundaberg/Wide Bay region only)
Our Service	The Aged Care Navigation service provided by ADA Australia is: Independent and free from conflict of interest Confidential Client Directed Free
Funding	The Aged Care Navigators Trial is delivered as part of a consortium led by COTA Australia and funded by the Australian Government. The Aged Care Navigator pilot is funded until June 30, 2020. For information about the Aged Care Navigator Trial Program go to www.AgedCareNavigators.org.au
Education	Community education sessions are available in the Bundaberg/Wide Bay regions. These educations provide interested groups with useful information on understanding and accessing the aged care system.
We cannot help with …	ADA Australia’s Aged Care Navigators service is unable to provide Financial or legal advice Support to access retirement villages Support to address complaints or concerns relating to aged care services you currently receive (however other ADA Australia advocacy services may be able to assist, please contact us) Where issues are out of scope, we will always try to provide you with a more appropriate referral.
