| What we provide | The Aged Care Navigation Service provides

| Eligible clients | ADA Australia’s Aged Care Navigator services are available to people residing in the Brisbane South and Bundaberg/Wide Bay regions, who need information about aged care options, or help to access aged care
supports. This includes older people, their families, friends and others who want to link with aged care supports.
The services are particularly aimed at helping people who:

| We can help with … | The Aged Care Navigation service supports, educates and assists people by informing and linking them with suitable aged care services. They aim to help local people build their own knowledge by guiding them through the steps from information to assessment to receiving care.

| Funding | The Aged Care Navigators Trial is delivered as part of a consortium led by COTA Australia and funded by the Australian Government. The Aged Care Navigator pilot is funded until June 30, 2020. For information about the Aged Care Navigator Trial Program go to www.AgedCareNavigators.org.au | ||
| Education | Community education sessions are available in the Bundaberg/Wide Bay regions. These educations provide interested groups with useful information on understanding and accessing the aged care system. | ||
